Title: 16 players 5 rounds of golf

Every year I have a golf tournement with old friends.  To determine who plays with who we draw cards.  The resut of this method is often we play multiple times with one old friend while not playing with several others.  
This year I was hoping to come up with a table that we could use to allow everyone to play with different people the first 3 or 4 rounds but I have been stumped.  I get many duplicates starting in the 3rd round.  
If someone had a table that breaks this down for me it would be great!
Can anyone help?

Are you and your friends playing in foursomes?

If you are then this schedule will be exactly what you want.
