Round Robin Schedule was supposed to be as follows:
 DAY      DATE       BD # 1       BD # 2      BD # 3      BD # 4       BD # 5       BD # 6
Mon      23-May      10 @ 11      2 @ 9       6 @ 8       3 @ 7        1 @ 5        4 @ 12
Mon      6-Jun        3 @ 8        5 @ 11      4 @ 9       10 @ 12      6 @ 7        1 @ 2
Mon      13-Jun      5 @ 9        3 @ 12      1 @ 7       4 @ 8        2 @ 11       6 @ 10
Mon      20-Jun      4 @ 6        8 @ 12      2 @ 10      1 @ 11       3 @ 9        5 @ 7
Mon       27-Jun      1 @ 9        3 @ 10      7 @ 12      5 @ 6        2 @ 8        4 @ 11
Mon      11-Jul      2 @ 5        4 @ 7       3 @ 11      9 @ 10       6 @ 12       1 @ 8
Mon      18-Jul      7 @ 8        6 @ 11      4 @ 5       2 @ 3        1 @ 10       9 @ 12
Mon      25-Jul      2 @ 6        1 @ 4       8 @ 10      7 @ 9        4 @ 12       3 @ 5
Mon      1-Aug       1 @ 12       8 @ 9       3 @ 6       2 @ 4        5 @ 10       7 @ 11
Mon      8-Aug       1 @ 3        2 @ 7       5 @ 12      8 @ 11       10 @ 11      6 @ 9
Mon      15-Aug      2 @ 12       7 @ 10      9 @ 11      5 @ 8        3 @ 4        1 @ 6

However, week one Team 4 played 10 and 11 played 12.  This was not noticed until this past week (July 18th)

The following matches still need to be played:

Mon      25-Jul      2 @ 6       1 @ 4       8 @ 10      7 @ 9       4 @ 12      3 @ 5     (Team 4 listed twice.  Team 11 not listed)
Mon      1-Aug       1 @ 12      8 @ 9        3 @ 6       2 @ 4       5 @ 10      7 @ 11
Mon      8-Aug        1 @ 3       2 @ 7       5 @ 12      8 @ 11      10 @ 11     6 @ 9    (Team 11 listed twice.  Team 4 not listed)
Mon      15-Aug      2 @ 12      7 @ 10      9 @ 11      5 @ 8       3 @ 4       1 @ 6

Is it possible without having someone double up and someone else not ever play for a week?

It is possible to fix your schedule by completely rescheduling the last 4 rounds..   For example you could play:

 2 @  6    1 @ 12    7 @ 10    9 @ 11    5 @  8    3 @  4
 1 @  4    7 @  9    3 @  6    5 @ 10    8 @ 11    2 @ 12
 8 @ 10    2 @  4    7 @ 11    1 @  3    5 @ 12    6 @  9
 4 @ 12    3 @  5    8 @  9    2 @  7   10 @ 11    1 @  6

This is one of exactly 16 ways of solving the problem!
